Welcome to Smiles by Lorino (“we,” “our,” or “us”). These Terms and Conditions govern your use of our services, website, and any interactions you have with our practice. By scheduling an appointment or using our services, you agree to the following terms:

1. Appointments & Cancellations

Appointments must be scheduled in advance and are subject to availability.
We require at least 12 hours’ notice for cancellations or rescheduling.
Missed appointments or cancellations without proper notice may result in a cancellation fee.

2. Financial Policy & Payment

Payment is due at the time of service unless prior arrangements have been made.
We accept [list accepted payment methods, e.g., credit cards, insurance, cash, etc.].
If using insurance, it is the patient’s responsibility to verify coverage and benefits.
Any unpaid balances must be settled before additional treatment is provided.

3. Insurance & Billing

We will assist in filing insurance claims, but patients are responsible for any unpaid portions.
Insurance benefits are an agreement between the patient and their provider; we do not guarantee coverage.
Patients must inform us of any changes in insurance coverage before treatment.

4. Treatment & Consent

Orthodontic treatment involves potential risks, including but not limited to discomfort, changes in tooth position, and the possibility of requiring additional treatment.
Patients or guardians must sign an informed consent form before treatment begins.
We reserve the right to discontinue treatment if a patient fails to comply with prescribed care or payment terms.

5. Orthodontic Appliances & Maintenance

Patients must follow all care instructions for orthodontic appliances (e.g., braces, aligners, retainers).
Lost or damaged appliances may result in additional fees for replacements.
Failure to adhere to treatment guidelines may impact the effectiveness and timeline of treatment.

6. Privacy & Data Protection

We comply with HIPAA and other applicable privacy laws to protect your personal and medical information.
Patient records are confidential and will not be shared without consent, except as required by law.
Patients have the right to request copies of their records subject to administrative fees.

7. Limitation of Liability

We strive to provide high-quality orthodontic care; however, we do not guarantee specific results.
Patients assume all risks associated with treatment and agree to hold us harmless from any claims related to unforeseen complications.
In the event of dissatisfaction, we encourage open communication to find a resolution.
